Some people look tired no matter how much sleep they get. Concealer settles into the crease by mid-morning, photographs exaggerate the shadow, and no eye cream seems to shift it.

Very often this has nothing to do with rest. It comes down to a natural hollow beneath the eye called the tear trough, and as it deepens, it casts a shadow that everyone reads as fatigue.

What Is the Tear Trough?

The tear trough is the hollow that runs from the side of the nose, curving beneath the eye and above the upper cheekbone, creating a visible dip in the face under the eyes. It sits in the same region where eye bags, fine lines and wrinkles tend to appear.

The tissue here is exceptionally thin and delicate, thinner than anywhere else on the face, which makes it one of the first areas to show signs of ageing.

Tear troughs develop as we age and lose fat in the mid-cheek and under-eye area, leaving the hollow looking sunken. The result is a tired, older and less lifted appearance.

For some people, tear troughs are simply genetic and visible from their twenties. In either case the concern often worsens over time, as the skin beneath the eye thins and the blood vessels underneath become more visible through it. This is one reason the area can look dark as well as hollow.

Tear troughs VS eye bags

These two concerns are frequently confused, and the distinction matters.

A tear trough is a depression, a loss of volume. It responds well to filler.

An eye bag is a protrusion, where fat pads push forward through the lower lid. Filler does not remove a bag. In some cases, where a bag looks more prominent because of the hollow sitting directly beneath it, smoothing that hollow can make the bag far less noticeable. But if the bag is the primary issue, a surgical option such as lower blepharoplasty is usually the more appropriate route, and we will say so at consultation.

What Does Tear Trough Filler Treat?

Tear trough filler is well suited to:

• Hollowing and a sunken appearance beneath the eyes
• Under-eye shadowing and dark circles caused by that hollowing
• A persistently tired appearance despite adequate rest
• Loss of definition between the lower eyelid and cheek
• Mild eye bags accentuated by an underlying hollow

It is often combined with cheek filler, which lifts and supports the mid-face and gives the under-eye a stable foundation to sit on. Treating both together frequently produces a more natural, balanced result than treating the under-eye in isolation.

Who Is a Good Candidate?

You are likely to be well suited if you have genuine hollowing beneath the eyes, reasonable skin quality and elasticity, are in good general health, and have realistic expectations of a subtle, refreshing change rather than a transformation.

Tear trough filler is less suitable for those with prominent fat pads or significant eye bags, malar festoons or chronic fluid retention, very thin or lax under-eye skin, active infection in the area, or thyroid eye conditions.

It is not available during pregnancy or breastfeeding.  It is also illegal in England to administer dermal filler for cosmetic purposes to anyone under 18.

What Happens During a Tear Trough Treatment?

• Consultation:  Every treatment begins with a full facial assessment, medical history and photographs. We discuss what you would like to change, examine the anatomy, and give you an honest view of what filler can achieve. If it is not the right treatment for you, we will talk you through the alternatives instead.
• Preparation: Numbing cream is applied to the area and left to take effect. Most of the products we use also contain lidocaine.
• Treatment: We typically use a cannula rather than a needle in this area. A cannula has a blunt tip and is introduced through a single entry point per side, which reduces bruising and is widely considered to lower the risk of vascular complications, though no technique eliminates risk entirely. Small volumes are placed with care, usually between 0.5ml and 1ml in total across both sides.
• Duration. Most appointments take 30 minutes.
• Aftercare. Avoid makeup for the rest of the day, and skip heat, alcohol and strenuous exercise for 24 to 48 hours. Sleeping slightly elevated for the first few nights helps limit swelling. We schedule a review appointment at two to four weeks to assess the settled result.

Risks and Side Effects of Tear Trough Fillers

Under-eye filler is a high-risk area, and any clinic offering the treatment should advise on patient risk and safety.

Common and temporary: Swelling, bruising and tenderness, typically settling within three to seven days. Small lumps or slight unevenness in the first fortnight are common and usually resolve as the product settles.

Less common:  Prolonged puffiness, slight migration of product, or the Tyndall effect, a faint bluish-grey tinge that can occur if filler is placed too superficially. Both can be corrected.

Rare but serious:  Infection, and vascular occlusion, where filler blocks or compresses a blood vessel. Because of the vascular anatomy around the eye, this carries a very small risk of visual complications. It is the single most important reason to choose an experienced, medically qualified injector for this area.

How to Choose a Tear Trough Practitioner in London

Wherever you go, these are the questions worth asking:

• Are you registered with the GMC, GDC, NMC or GPhC?
• How many tear trough treatments do you perform each month?
• Which product will you use, and why that one for this area?
• Will you inject with a needle or a cannula?
• Do you keep hyaluronidase on site, and what is your complications protocol?
• Who do I contact if something concerns me out of hours?

Any competent injector will welcome these questions. Be cautious of clinics offering unusually low prices, time-limited discounts or same-day treatment without a proper consultation. The under-eye is not the place to shop on price.
